List of Entry Requirements
- Clean Camiguin QR (please visit www.cleancamiguinqr.com to register)
- Proof of Vaccination (Vaccination Card or Vaccination Certificate)
- Confirm (confirmation slip from your booked LGU-accredited tourist accommodation)
- Health Declaration Form -HDF- (find the HDF on the main page of the website)
- Traze App (required by the Phil. Ports Authority. Available at www.traze.ph)
Which is the Procedure?
Please prepare the following on your device (mobile or computer):
- Latest picture of your face
- Photo of your ID
- Your photo holding your ID beside your face
- Booking Confirmation
- Picture of your Vaccination Card (a vaccination card is necessary also for the 5 years old child or above)
How to register:
- Go to https://cleancamiguinqr.com
- Select VISITOR.
- Select between FIRST TIME VISITOR or RETURNING VISITOR.
- Fill up the form with all your details. Click on NEXT and your QR will be generated.
- Back to the main page.
- Login with your QR and password.
- Choose VACCINE INFORMATION from the drop-down menu
- Fill up the form with all the details of your vaccination card
